Independent Contractor Required Paperwork for Contracting

All drivers applying to be a company driver must apply using the Company Driver Application button below.

This application process is for tractor(s) only. Independent contractors are required to apply using the button below in addition to scheduling a tractor inspection appointment and uploading all required documents. The application is not complete until all applicable forms are submitted and a tractor inspection training has been attended.

These forms can either be printed off by you and taken with you to your inspection appointment or can wait and you will receive them from the terminal manager during your inspection appointment:

Please select a time for a 1 hour inspection appointment using the calendar below. Once you select a time, you will be prompted to input your personal information and after given the opportunity to upload the necessary forms.

Here is a list of the forms you will need to upload:

Title or Registration (Required)
Form 2290 Current Year (Required)
Direct Deposit Bank Authorization Form or VOID Check (Required)
Annual Inspection (Required)
Last Preventative Maintenance (Required)
Empty Scale Ticket or Proof of Vehicle Weight (Optional if not listed on title or registration)
Cab Card (Optional if you have your own plates)
Insurance (Optional if you have your own insurance)
Front of CDL (Optional if you want to purchase insurance from our company)

As mentioned above, the application is not complete until all applicable forms are submitted and a tractor inspection training has been attended.
